Name of the Course:
  • G.N.M (General Nursing and Midwifery) Training Course
  • A.N.M. (Auxiliary Nurse and Midwives) Training Course
GNM Nursing Eligibility Criteria:

For ANM:
  • Applicants must have completed 10+2 Standard in Arts (Mathematics, Physics, Chemistry, Biology, Biotechnology, Economics, Political Science, History, Geography, Business Studies, Accountancy, Home Science, Sociology, Psychology and Philosophy) and English Core/English Elective or Science or Health care Science – Vocational stream ONLY passing out from recognized Board.
  • As per GNM notification, National Institute of Open Schooling (NIOS) candidates are also eligible to apply.
For GNM:
  • Applicants must have passed 10+2 with PCB and English as compulsory subject.
  • They can also apply with vocational stream subjects from recognized board.
  • 40% marks are necessary marks in the qualifying exam.

Age Limitation:
  • Candidates who are going to apply for GNM / ANM Nursing Course must be 17 years of age or above as on 31.12.2018.
  • Relaxation in age will be allotted to the reserved category candidates as per the directives of the Government of India
Application Fee:

According to GNM nursing notification, applicants are required to submit the application fee of Rs. 300/- through SBI Challan.

Selection Criteria:

Candidates will be selected for G.N.M / A.N.M Training Course by the marks obtained in compulsory subjects in H.S. (10+2)/ equivalent Examination.
The Govt in Health & Family Welfare Department is going to organize Document Verification at Nodal Centers to provide admission in GNM and ANM Courses.

Ways to fill GNM Nursing Application Form:

Step1) Online Registration
  • First of all, candidates have to log on to the official website of Govt in Health & Family Welfare Department that is “www.dohodisha.nic.in”.
  • Now press on the “Download>>Prospectus” tab available on top side of the homepage.
  • Then hit on the appropriate link of notification and read it very carefully.
  • After that, candidates need to hit on “Apply Online” link and select “ANM / GNM” link.
  • Now you must fill GNM nursing application form 2018 19 including detailed personal information in correct manner.
  • Hit on the save button and user id and password will be generated and after that you can follow the further procedure.
Step2) Online choice filling
  • Candidate must verify the name of the institutions & total no. of seats available in them before filling the choices.
  • Candidates are advised to give maximum numbers of choices in order of preference.
  • Final Choice locking must be completed before Document Verification at the Nodal Centre.
Step3) Document Verification
  • Attach all the vital testimonials and certificates along with the ANM application form at provided place.
  • Candidates must submit any of the below given original documents along with the form.

Step4) Publication of Provisional Merit List & 1st Round Seat Allotment
  • Merit list will be prepared on the basis of career marks.
  • Seats will be allotted on the basis of the merit list.
Step5) Depositing Part course fee for 1st Round Seat Allotment
  • After this candidates have to deposit the Course Fees of Rs. 5000/-.
  • The Candidates will pay the Rest amount of the Total Course Fees during reporting / admission at the Concerned Institutions.
Step6) 2 nd Round Seat Allotment & Deposition of Part Course Fees
  • The Candidates, who have been freshly allotted seats in this Round, need to deposit the Part Course Fees at the Nodal centers.
Step7) 3 rd Round Seat Allottment & Deposition of Part Course Fees & Reporting at the Institutions
  • Candidates who have been allotted seats in this round have to pay the application fee.
  • All the Allotted Candidates who have paid the Part Course Fees are requested to Report at the Institutions on the Scheduled Date as mentioned in the Intimation Letter.

Documents to Be Attached With GNM Nursing Application Form:
  • Copy of the submitted application form duly signed by the candidate or a copy received from the help centre.
  • Original 10th (HSCE) pass certificate and mark sheet issued by BSE Odisha or equivalent thereof.
  • Original 10+2 pass certificate and mark sheet issued by CHSE Odisha or equivalent thereof. Original College Leaving Certificate / Transfer Certificate
  • Original character /conduct certificate from the Head of the Institution last attended.
  • Domicile/ Residential / Nativity Certificate from competent authority.
  • Authentic proof of passing Odia as a subject at M.E / HSCE issued by Board of secondary education or other equivalent board (for candidates applying for Govt. Institutions & 85% of Govt. quota seats of Private Institutions.
  • Original caste certificate (SC/ST candidate) issued by the competent authority.
  • Physically Handicapped certificate (in original) issued by the competent medical authority of the District (in case of Physically Handicapped candidates only).The percentage of disability, locomotor disability of lower limbs between 40% to 50% only will be taken into consideration.
  • Original SBI Challan deposit slip, Rs.500/- (Rupees Five Hundred) only towards application fees.
  • Two copy of self-attested recent passport size Colour photograph.
  • Original certificate of Serviceman/Ex-Serviceman from Rajya Sainik Board (Annexure- IV).
  • Original Green Card (with all pages) issued by the Chief District Medical Officer, (In case of candidate claiming for the seat under green card quota). The name and Date of birth in green card should match with the name and Date of birth mentioned in the HSCE/equivalent certificate. Any difference regarding this should be supported by an affidavit duly sworn in before the executive magistrate.
  • The original Medical Fitness certificate issued by the Govt. Allopathic medical office
